A six-month, food-first diabetes pilot built to produce a real proof point.
250 eligible members identified. Approximately 100 engaged. Twenty-six weeks of weekly groceries, coordinated clinical care, practical education, and a formal Month 6 evaluation.
A focused place to begin.
Diabetes 250 is designed for people with diagnosed diabetes and at least one additional chronic condition. The eligible population can then be prioritized using clinical risk, utilization, social barriers, and available claims data.
Food funding follows completed grocery deliveries.
Condition-supportive food connected to the care plan.
Physician-led oversight and registered dietitian care.
Shopping, cooking, labels, medications, and real-life eating.
Agreed measures, Month 6 outcomes, and a maintenance plan.
Simple, utilization-based structure.
Partners fund the incremental food intervention based on completed grocery deliveries. Eligible clinical services can flow through existing medical benefits when coverage and reimbursement requirements are met.
Clinical learning shapes the groceries that follow, so members practice new behaviors at home each week.
Comparison methods and outcome measures are agreed before launch. Diabetes 250 does not promise guaranteed clinical improvement, savings, utilization reduction, or claims ROI.
