They sit at the highest clinical intensity of the Food Is Medicine spectrum and are typically used post-hospital-discharge or for members who are chronically homebound — when the priority is removing every barrier between a person and the food their care plan calls for.

Unlike a generic meal-delivery service, medically tailored meals are tied to your diagnosis and your care team. A dietitian reviews your condition, medications, and discharge plan, and every meal is built to match — so the food that arrives at your door and the plan in your chart finally agree.
