Pennsylvania's Bridges to Success: Keystones of Health is a five-year Section 1115 Medicaid demonstration (January 1, 2025 – January 1, 2030) that addresses health-related social needs across four focus areas: reentry, housing, food and nutrition, and continuous coverage for young children.
The Food and Nutrition component provides direct food support — including medically tailored meals or groceries — to specific Medicaid populations facing food insecurity, with a goal of connecting members to longer-term assistance like SNAP. Services are available to members who have both a clinical need and an assessed social need.
